Accountable Care Organization (ACO) Information

Annapolis Internal Medicine, LLC  is a part of an Accountable Care Organization, otherwise known as an "ACO".   Medicare describes ACO's as "a group of doctors, hospitals, and/or other health care providers who work together to improve the quality and experience of care you get.

These organizations help your doctors and other health care providers understand your health history, and talk to one another about your care and your health care needs. This can save you time and money by avoiding repeated tests and unneeded appointments. It may make it easier to spot potential problems before they become more serious—like drug interactions that can happen if one provider isn’t aware of what another has prescribed.

ACO providers communicate with each other and partner with you in making health care decisions. They share information and may use Electronic Health Records (EHRs), so you'll likely have fewer repeated medical tests and may save time on paperwork."

If you don’t want Medicare to share your health care information this way, call 1-800-MEDICARE (1-800-633-4227) and tell that to the representative. TTY users can call 1-877-486-2048. If you decide you don’t want Medicare to give your health care information to your doctors or other providers, Medicare will continue to use your information for some purposes, like evaluating financial performance and quality of care. 


Welcome to Medicare Physical and Annual Wellness Visits (AWV) Information

Medicare Part B covers a"Welcome to Medicare" preventive visit, if completed within the first twelve months (12) months of enrollment with Part B.  This visit is covered 100% by Medicare and your annual deductible does not apply.  Keep in mind, if your provider performs additional tests or services, you may have a co-insurance for those services.  More information about the Welcome to Medicare preventive visit can be found here.

If you've had Medicare Part B for longer than twelve (12) months, you can have an "Annual Wellness Visit" to develop or update your personalized plan to help prevent disease based on your health and current risk factors.  Again, if your provider performs additional tests or services, there may be a co-insurance for those services.  More information about Annual Wellness Visit coverage can be found here.

​Maryland Primary Care Program (MDPCP)

Annapolis Internal Medicine is also part of the Maryland Primary Care Program (MDPCP).  MDPCP is voluntary program open to all Primary Care practices with primary care providers.  The program provides resources to your primary care team to better manage and coordinate care, with no extra cost to our patients.  More information about the MDPCP can be found here.
