However, if you prefer not to use the online portal, please use the links at the bottom of the page to complete and applicable forms. You must disable your pop-up blocker for this section to work.
PLEASE NOTE: Annapolis Internal Medicine is happy to partner with ChartRequest for processing all medical records requests. This includes patients, other medical practices, and third-parties (law practices, insurance companies, court proceedings, etc). ACUTE CARE VISITS IN THE CLINIC
As part of our ongoing effort to provide timely and personalized care, we’re making a small adjustment to how acute visits are handled.
Our Acute Care Clinic remains open and ready to help when you need prompt attention. We’ll continue to accommodate walk-ins as availability allows, but scheduled appointments take priority.
